Hold Harmless Agreement

participation in the SPRING HILL LIONS CLUB SUMMER BALL PROGRAM during the __________ season and do hereby assume full responsibility for all risk and hazards incidental to such participation, including transportation to and from the activities: and I/we hereby waive, absolve, indemnify, and agree to hold harmless the SPRING HILL LIONS CLUB, the organizers, sponsors, supervisors and participants.
